Cost-effectiveness analysis of kidney transplantation versus dialysis in Ukraine: a comparative study

Abstract

This study evaluates the cost-effectiveness of kidney transplantation compared to hemodialysis for treating end-stage renal disease in Ukraine. Using Markov modeling, the analysis assessed costs, quality-adjusted life years (QALYs), and incremental cost-effectiveness ratio (ICER) over a 10-year horizon. Results showed that kidney transplantation provided 1.9 additional QALYs per patient compared to hemodialysis. The ICER for transplantation was $4,462 per QALY, falling within the World Health Organization's threshold for cost-effectiveness. Sensitivity analysis confirmed the economic viability of transplantation, even with potential cost increases for the government. Although transplantation had higher initial costs, it resulted in better survival rates and quality of life outcomes. The study concludes that prioritizing funding for kidney transplantation is strongly advised in Ukraine, as it is a cost-effective approach for managing ESRD. Limitations include the assumption of ideal transplantation conditions and the exclusion of comorbidities in survival estimates.
